To streamline your first visit, we invite you to fill out our New Patient Documents. You can either print them out and bring them completed to your first visit OR you can fill them out online. (We recommend that you submit the completed forms online to expedite your check-in process when you arrive for your appointment.)
Upon arrival, our friendly staff will be here to greet you and guide you through the check-in process. Please bring your new patient paperwork and any relevant past medical records. If your pet is not friendly with others, we recommend that you enter initially without your pet so that we can ensure the space is safe for your pet to enter. At our clinic, we can provide a calm environment with secluded exam areas and gentle handling to help anxious pets feel safe and secure. During your first appointment, one of our doctors will take the time to get to know you, discuss your pet’s history, and address any questions or concerns you may have.
